The Little Bookworm: Enhancing Speech, Language and Literacy includes an independent speech pathology practice under the ownership of Michelle O'Brien.

Parents and guardians of children receiving services from The Little Bookworm have the right to gain access to the information held by us about their children.
The Little Bookworm needs to collect information about each child receiving service for the primary purpose of providing quality treatment. In order to fully assess, diagnose and treat children, we need to collect some personal information from the parents and/or guardians of each child. This information will also be used for the administrative purposes of running the practice such as billing parents/guardians, the child's school or an insurer. Information will be used within the practice for handover if the need arises for another speech pathologist to provide children with ongoing assistance.
The Little Bookworm may disclose information regarding diagnosis or treatment to a child's doctor or other treatment providers only with parental/guardian consent. The Little Bookworm will not disclose a child's information to commercial companies however specific service or product information as deemed suitable for your speech pathology management may be forwarded to parents/guardians by us, unless they instruct The Little Bookworm not to forward this type of information. Parental/guardian written consent will be obtained at the start of your treatment in order to carry out the above activities.
Information at The Little Bookworm is stored securely and only practice staff has access to it. The Little Bookworm takes all reasonable steps to ensure that information collected about each child is accurate, complete and up to date. Parents/guardians may have access to their child's information on request and if they believe that any of the information is inaccurate we may be able to amend it accordingly. If parents/guardians do not provide relevant personal or health information, in part or in full, to The Little Bookworm it may result in an incomplete assessment. This may impact on the diagnosis and the following therapy that is provided.
All staff working for The Little Bookworm have a current police check and working with children card, that allow them to work with children, within any State or Territory in Australia.
